Position Overview:

The Team Leader is responsible for population management, cross-system consultation, provider management and treatment quality monitoring and utilization management as needed. The Team Lead is responsible for overseeing or managing a caseload of providers and schools as needed to ensure that members have access to effective behavioral health services and programs are being implemented to fidelity. This is done by overseeing and attending cross systems meetings, providing professional development sessions, population management, cross-system consultation, relationship management, provider management and treatment quality monitoring. The Team Lead must utilize clinical knowledge of best practices and evidence-based treatments to ensure CBH members receive timely access to quality, medically necessary behavioral health services. When clinically indicated, School Based Services Liaisons consult with CBH Clinical Care Managers, Physicians and Psychologist Advisors to ensure the most effective treatment for the CBH member is being authorized.

Essential Functions:

  • Provides leadership to the team in conjunction with or in the absence of the team Supervisor.
  • Completes essential functions of a Clinical Care Manager.
  • Trains and orients new Clinical Care Managers, CBH Staff and Visiting Professionals.
  • Creates and maintains team documents which may include schedules, routine data collection and team organizational tools.
  • Makes grievance assignments; notify outcomes of grievances and other administrative tasks, as needed.
  • Consults with physicians regarding denials, length of stay and psychiatric evaluations, as needed.
  • Meets with other team leaders and collaborate with different systems and agencies via telephone, email and in meetings.
  • Participates in interviews for new hires.

Team Specific Responsibilities:

  • Support work of the school-based services liaison to ensure access to quality behavioral health supports for students that need tier 3 interventions from CBH providers.
  • Attend and participate in cross systems meetings with schools and their assigned providers.
  • Engage in outreach and communication with parties that include counselors, STEP teams, principals, provider staff, CBH internal staff, P&I staff, other levels of care providers, and other entities involved in the coordination of care.
  • Attend monthly provider meetings as needed.
  • Complete regular documentation which includes notes in MCO, meeting notes/minutes, SBT internal checklist and audit tool, documentation of concerns, and quality indicator reports.
  • Lead professional development sessions for cross systems and community partners related to school-based programming.
  • Attend a variety of different trainings for own professional development which include clinical, administrative, and professional topics.
  • Support in monitoring component of MOU implementation in school including assisting Clinical Coordinator with Quick Base data reporting for School Based Services Liaison follow up.
  • Assist with critical incidence response as needed.
  • Manage and coordinate care by conducting telephonic and on-site reviews, obtaining clinical information to determine medical necessity for authorization of care and helping to guide aftercare planning and reviewing evaluations, as needed.
  • Meet with other team leaders and collaborate with different systems and agencies via telephone, email and in meetings.

Position Requirements:

  • Education: Masters degree in Social Work or related field or Registered Nurse
  • License/Certification: License eligible and willing/able to obtain licensure within the first year of employment; PA Child Abuse Clearance required
  • Relevant Work Experience: Minimum of 2 years of post-graduate, direct clinical care experience working with adults, children, adolescents and their families in behavioral health settings and experience in assessment and treatment planning

  • Excellent time management and prioritization skills and ability to multi-task
  • Compliant with HIPAA regulations
