You are invited to join the VNA hospice team as a hospice/palliative care nurse practitioner.

Position Summary: In this role, you will work in collaboration with the chief medical officer and other members of interdisciplinary group (IDG), and palliative care team, in an active practice to deliver hospice, palliative care and related health care services to patients and families within the inpatient unit (IPU), long term care (LTC) facilities and the home hospice program.

Essential Functions:

  1. Treats hospice and palliative care patients, including treatments and other therapeutic or corrective measures, within the nurse practitioner’s scope of practice.
  2. Completes comprehensive history taking, physical exams, as well as prescribing tests as needed and other adjunct treatments and therapies for patients and documents in home care home base emergency medical records system.
  3. Prescribes medications as appropriate and licensed.
  4. Completes Hospice F2F visits and documents these visits as per Medicare requirements
  5. Utilizes assessment, planning, intervention, and evaluation to implement the IDG care plan, also serves as a resource to patients, their families and team members.

Knowledge and Experience:

  1. Registration as Nurse Practitioner in the Commonwealth of Pennsylvania.
  2. One year’s experience in provision of relevant direct patient care.
  3. One Year Hospice experience
  4. Valid driver’s license and auto insurance.
  5. DEA license and prescriptive authority.
  6. Preferred experience in program development

Education: Master’s Degree in Nursing with focus on Nurse Practitioner training and responsibilities.
